•  Some action icons have not been updated to use row specific alt text. 32309
•  The icon used for a no login error contains text. The icon and text should be separate for accessibility and localization needs. 26992
•  The option to save a quiz answer appears in the header, rather than next to the question. 30613
•  The Quizzes and Discussions tools do not use the Heading 1 attribute consistently.  30918
•  Toggle controls (checkboxes) do not have descriptive text communicating their current state. 9038
